For your convenience, you can now complete your welcome form before coming to our office:

Click here and you will be redirected to the secure online site where you can complete your welcome form. 
Begin with your Personal Information, and complete each section.  When finished, simply click "Done" in the top right corner, and your welcome form will be e-mailed into our system.

For your convenience, when completing the ocular and health history you can set all responses to "NO,"
and then change the few responses to "YES" as necessary.
